8.      In case of Rented premise, Notarised Rent agreement with Rent receipt.  

Including NOC (No- Objection Certificate) from the landlord.

9.      Educational Qualification along with Experience letter of the Registered Pharmacist and Appointment Letter.

10.  Invoice of Refrigerator in the name business.
